Dear Prescribers


When filling out a prescription for a controlled substance, please note the area that the red arrow is pointing to. This is the ADDRESS of the patient, and this is REQUIRED to be a valid and legal prescription. See this LINK.

Your attention to filling out this part of the prescription is most appreciated.

The reason that most prescribers won't do this is that the onus is on the pharmacist and pharmacy to have it correct. It's like giving a ticket to the person in the back seat for the driver speeding. Until they hold prescribers accountable for all their actions nothing will change. I would love nothing more than to give the prescription back to the patient and tell them that they have to go back to the prescriber and have them finish filling it out. But that day will never come.

But ... I'm a doctor. Those rules don't apply to me...... (said with an ample dose of sarcasm)
I work in imaging. Just try getting a clinical history -- which physicians are required to provide.
